A healthy 55-year-old woman attends the appointment with an almost total absence of maxillary teeth. After evaluation of the complementary exams, it was confirmed that there was horizontal bone loss, as well as bilateral pneumatization of the maxillary sinus. The patient is a healthy 39 year old woman. The crown on tooth #16 (3) broke that day while biting bread. There was no way to rescue the tooth. She has pain and discomfort. Tooth #16 (3) was extracted without flap reflection and the defect was filled with 3D Bond™ 0.5 cc, and a collagen sponge. Bone regeneration was observed both clinically and in radiographic images 3 months post-op, and the ultrashort Bicon implant was placed (D-4.5mm, L -6.0mm). Final prosthetic restoration was done 4 months later with a full ceramic crown. In the next 12 months no inflammatory symptoms were observed.

Activate the syringe and wait 1 minute before application. Eject the cement into the sinus cavity through the sinus lateral window until 2/3 of the sinus is filled.  For filling the last 1/3 and closing the sinus window. After activation of the cement (Do not wait 1 minute, eject it immediately into the site, place sterile dry gauze, press firmly for 3 seconds, and close the flap.

The cyst appeared 4 years after successful endodontic and prostetic treatment. The first clinical symptom was an abscess. Three weeks after the abscess incision, we completed a one step endosurgery treatment with a root resection of tooth #12 (7), cyst enucleation (with histopathology examination), and backfill of the root canal with MTA. In addition, we used dedicated microtools and then filled the bone defect with Bond Appetite® (2 cc).

The implant in tooth #13 (6) is of 3 mm in diameter, the other implants #14 (5), #15 (4) and #17 (2) are of 3.75 mm in diameter.

Augmentation of the maxillary sinus when severe atrophy and/or pneumatization of the sinus has occurred results in minimal crestal height that will not allow simultaneous implant placement has been a clinical challenge. The lateral sinus augmentation approach has been traditionally utilized in these clinical situations, but there are known complications such as membrane tears and increased post-operative issues that follow use of this technique. This article aims to propose a new technique utilizing specific surgical instruments, making it possible to increase the height of the crestal bone under the maxillary sinus in cases of significant atrophy, without complications using a two-stage crestal approach with biphasic calcium sulphate as the graft material. The article will discuss the technique, present a case example and review 51 cases representing initially 1-3 mm sub sinus crestal bone heights that the technique was utilized with.

Use of a Biphasic Calcium Sulphate Graft to Treat Posterior Maxillary Atrophies by Avoiding a Lateral Sinus Approach
Extraction of the natural tooth may be a prelude to implant placement. This may be done using an immediate placement protocol or require a delayed approach depending on multiple factors that include residual infection related to the failed tooth being extracted, availability of bone to stabilize the implant at placement, or soft tissue issues. Socket preservation is recommended when the delayed approach is selected to create an osseous bed with adequate height and width that can accommodate the implant that is planned.
